To submit your Claim online, you will need the following information to verify your identity:
- The Phone Number that You Received a PreRecorded Call from PRA and/or Wal-Mart
- Notice ID (located on the Postcard Notice you received) & Your 9-digit PIN (located on the Postcard Notice you received).
Your online Claim must be submitted on or before 11:59 PM (Eastern) on March 9, 2020.
- Once completed, a copy of your online Claim form will be emailed to you.
- You should NOT submit both an online claim AND a paper form. Only one Claim form will be accepted per qualified individual.
